    Hi Anthony,

    We have figured out that the issue is occurring because you are using text input for the image answers. Perhaps, that’s why it isn’t working for you.

    We suggest you, set up a quiz using rich text answer with an image in it.

    Please see our example screen-cast. The image selected is shown in quiz results.

    I just performed a test survey on your website by referring to the video we just mentioned above. And I was able to collect results. I suggest you to please setup the recent survey as we have described in the video.
