    Just recently, probably after the last update, we started having issues on quizzes with random questions being marked as wrong even when they are correct. Please see the image sample that shows what the test takers are seeing. I don’t know if this has to do with %QUESTION_ANSWER_INCORRECT%, but it also scores the answer as wrong – not just the display. We have several tests and as said, it happens randomly between them – all tests are multiple choice and there is no reason we can see as to why it’s happening.

    The site is goodfeettraining.com and the password to enter is G00dF33t.
    Each chapter has a test at the end – you can find them in the mega menu in the nav.

    Example:

    Chapter 2
    Answers missed:

    Good Feet has licensed physicians in all of their locations.

    Correct Answer: False
    Answer Provided: False
    Chapter 4:

    How many steps are there to the fitting process?

    Correct Answer: 7
    Answer Provided: 7

    Chapter 1:
    Answers missed:

    Which is the largest bone of the foot?

    Correct Answer: The calcaneus
    Answer Provided: The calcaneus

    Hi Skessler,

    Please provide me the admin credentials of your website so that I can check whether the issue is happening because of %QUESTION_ANSWER_INCORRECT% template variable or not?

    Make sure to set the message as private before sharing the credentials.

    I’ve seen this happen in Quiz And Survey Master when answer formatting or hidden characters cause the system to read a response differently than expected. Sometimes trimming spaces or re-saving the question fixes it. In cases like this, reviewing how the values are stored usually reveals the mismatch.
