    How do I remove the username and date from the survey?

    Hi,

    This is not coming from a QSM quiz. This is coming from a WordPress blog post because it seems like you have pasted QSM shortcode on the blog post. This seems default and coming from the WordPress theme. Because QSM does not display Username & Date on its own on a webpage.
