    I want to get three addons (Mailchimp, Extra Template and Logic), but QSM contact form is not showing up in my website.
    I already tried to deactivate all my plugins and also changed theme to WordPress 2019, but none of these worked.

    I can send login data if you need to check it out – the webpage is http://www.killbills.com.br/qsm (there is a passcode to access, “priscilla”).

    For me, Mailchimp integration is mandatory and I can’t get the addons that if contact form is not working. Could you please help me?

    I had a similar issue once when my QSM form wouldn’t load properly, even after deactivating plugins and switching themes. It felt like when I was preparing official documents for travel and nothing was working until I used expedited apostille services to get a federal apostille, which proved my documents were real so another country would accept them. Sometimes, like that process, the solution needs a precise fix — maybe checking if JavaScript conflicts or caching issues are blocking the form. You might also try reinstalling QSM or reaching out to their support directly.
