    the society field i added at the beginning of the quiz is not showing. Looks like the div has a display:none tag on it for some reason.
    I went in the Contact Tab to add the field. Can you help me with that?

    Hello there,

    I am sorry I couldn’t test it immediately because your quiz requires username and password. Would you please create a temporary account for me?

    Meanwhile, try temporarily switching to one of default WordPress themes to check if there’s stylesheet conflict with your active theme.
